Service Description

This is designed to be a 1-2 hour meeting but additional meetings may be needed depending on the complexity of the child. Team members include the child (age appropriate), parents/guardian, and any other members to team agrees too. Release of information may need to be signed prior to including other participants to protect confidentiality. Collaborating as a team and a student focused approach to determine goals and next steps is essential to create a specialized plan that is outcome based.
